Have we reached a unified path forward for multiple sclerosis? Enter the new 2025 MS Criteria. Criteria are the agreed upon rules that health care providers will use to guide MS diagnosis and improve its accuracy. Toljan and colleagues describe in a new viewpoint in JAMA Neurology how the field has reached a major turning point in creating a unified diagnostic approach for MS. The approach brings biology, imaging and spinal fluid markers together to aid in diagnosing MS earlier and more accurately, and of course for helping to avoid missteps. Key Points: - The authors discuss the expansion to include the optic nerve. This inclusion improves sensitivity for MS diagnosis. - There is less reliance in the diagnosis for (using the old-fashioned) time and space criteria (DIT and DIS). - CSF markers and the central vein sign can be helpful in diagnosis. - Recognition of asymptomatic and non-specific presentations allow MS to be diagnosed even when symptoms are minimal, however careful exclusion of mimics remains essential. My take: We can learn a ton from MS and how the experts have established diagnostic criteria. When I was a resident the MS diagnostic criteria were clinical demonstration of dissemination in space (DIS) and time (DIT). We have moved well beyond this by carefully applying advances in the science. Here are 5 points that resonated w/ me: 1- Unified criteria simplify the landscape, however they demand careful application. 2- Earlier detection is now possible w/ new imaging markers such as the central vein sign and paramagnetic rim lesions. 3- Spinal fluid advances matter, as κ free light chain index may eventually substitute for oligoclonal bands. 4- Asymptomatic MS can now be diagnosed which means more folks may qualify for early intervention. 5- Excluding alternative explanations remains key, because misdiagnosis carries real consequences. https://jamanetwork.com/journals/jamaneurology/fullarticle/2841154 #michaelokun #fixelinstitute #parkinson #MS #multiplesclerosis sclerosis
