Key Points: - New hyperkinetic movements appeared within 48 to 72 hours of starting modafinil in three older stroke survivors. - Extensive testing was normal and no metabolic or structural cause explained the abnormal movements. - Stopping modafinil led to full resolution within 36 to 48 hours in all cases, highlighting what appeared to be a reversible reaction. My take: Modafinil is a wakefulness promoting medication most used to help folks stay alert when their natural sleep wake systems become disrupted. Narcolepsy, obstructive sleep apnea and shift work sleep disorder are the main uses for the drug. Clinicians should be aware of potential adverse effects when trying modafinil for an off-label indication like fatigue.
