Hyperkinetic means excessive and involuntary movements that arise when motor circuits in the brain become overly active. Javali and colleagues describe in a new paper in Movement Disorders Clinical Practicethat a group of post-stroke folks developed unexpected ‘choreiform and irregular movements’ shortly after starting modafinil. The series reminds us that medications used to boost wakefulness can have neurological side effects in vulnerable brains. Key Points: - New hyperkinetic movements appeared within 48 to 72 hours of starting modafinil in three older stroke survivors. - Extensive testing was normal and no metabolic or structural cause explained the abnormal movements. - Stopping modafinil led to full resolution within 36 to 48 hours in all cases, highlighting what appeared to be a reversible reaction. My take: Modafinil is a wakefulness promoting medication most used to help folks stay alert when their natural sleep wake systems become disrupted. Narcolepsy, obstructive sleep apnea and shift work sleep disorder are the main uses for the drug. Clinicians should be aware of potential adverse effects when trying modafinil for an off-label indication like fatigue. Here are 5 points that resonated w/ me from this case and this video: 1- Modafinil can rarely trigger involuntary movements in post stroke brains w/ fragile motor circuits. 2- Movements can look ‘choreiform or irregular’ however in these cases they stopped when the medication was withdrawn. 3- Health care providers should monitor closely when starting wakefulness agents in older adults recovering from stroke. 4- Stroke may have lowered the threshold for abnormal movements.
