
FDA Calls for New Safety Warnings on Seizure Risks for Specific Parkinson’s Medications

The US Food and Drug Administration (FDA) has issued a significant safety communication regarding medications containing carbidopa/levodopa. Following a detailed review of clinical data and safety reports, the agency is now requiring manufacturers to update their labels to warn that these treatments can lead to a deficiency in vitamin B6, which in some cases may trigger seizures.
This warning stems from an investigation into 14 documented cases where people with the condition experienced seizures directly linked to low vitamin B6 levels. Interestingly, the research showed that the risk is particularly high for those on higher doses; specifically, those taking more than 1,000 mg of levodopa daily. For those on doses exceeding 1,500 mg, the deficiency tended to develop even more quickly. These seizures often appeared as "focal onset," starting in one area of the brain before spreading, and in some severe instances, led to a continuous state of seizure known as status epilepticus.
The biological reason for this is quite specific: as levodopa is converted into dopamine in the body, it naturally depletes vitamin B6. Furthermore, carbidopa can actually bind to the active form of the vitamin, making it less available for the brain to use. Beyond seizures, a lack of B6 can show up in other ways, such as feelings of depression or confusion, inflamed lips or skin, and even "pins and needles" or muscle weakness caused by nerve damage.
The clinical evidence highlighted a very clear solution. Of the patients in the study who were given vitamin B6 supplements, every single one saw their seizures stop—even those who hadn't responded to standard anti-seizure medications. However, the risks of untreated deficiency are serious, as the review sadly noted two deaths among patients with documented low B6 levels and poorly controlled seizures.
In light of these findings, the FDA now advises healthcare professionals to check baseline vitamin B6 levels before starting anyone on carbidopa/levodopa. They also recommend periodic monitoring during treatment to ensure levels stay within a healthy range. If you are taking these medications, it is vital not to make any sudden changes to your dose. Instead, speak with your specialist about whether a simple B6 supplement might be a helpful addition to your routine to keep your levels balanced and your brain protected.
