
Research exposes severe shortage of non-drug therapy trials for advanced Parkinson's

A narrative review published in the Journal of Neural Transmission by Conor Fearon, Anette Schrag, and an international team of researchers reveals that clinical guidance almost completely ignores the later stages of the condition. While non-pharmacological and non-surgical interventions are widely accepted as crucial parts of overall care, standard guidelines treat Parkinson's as a single, uniform journey. They offer virtually no tailored directions for when complex symptoms take hold.
To evaluate the strength of the current evidence, the authors conducted a multi-step review of existing scientific literature. First, they gathered and examined recent systematic reviews of international clinical practice guidelines to see how often stage-specific advice was given. Second, they carried out a dedicated scoping review searching for all published rehabilitation trials that specifically recruited participants with advanced Parkinson's. Finally, they synthesised meta-analyses across individual symptom areas, ranging from swallowing difficulties and freezing of gait to pain, autonomic system dysfunction, and neuropsychiatric care.

Promising strategies like cognitive behavioural therapy for mood, non-invasive brain stimulation, targeted swallowing therapies, and specialised exercise setups remain largely untested in advanced cohorts.
The review emphasises that as mobility decreases, swallowing becomes harder, and care partner strain increases, relying solely on general recommendations is insufficient. The authors call for an immediate shift in research priorities to include advanced stages directly in clinical trials, ensuring holistic care that covers emotional wellbeing, caregiver support, and symptom relief alongside physical movement.
